Who handles your information?
Lifeline US is currently a project name, not a corporation, nonprofit, charity, or other registered legal entity. The individual or future entity responsible for the website must be identified here before launch.
Information you provide
The assistance request form may collect your name, email address, mobile number, city or community, province or territory, contact preference, general categories of need, request summary, Amazon.ca wishlist link, adult and Canadian-residency confirmation, contact consent, and optional SMS consent.
Please do not submit a street address, financial or banking information, government identification, passwords, medical information, children's identifying information, or other sensitive data.
Why the information is used
Submitted information is intended to be used to respond to your request, evaluate whether a possible introduction is available, communicate about the process, administer an introduction, address safety concerns, and maintain records of your permissions and preferences.
Form processing and cross-border handling
Form submissions are transmitted through Formspree, a third-party service. Depending on its systems and the location of the site operator or service providers, information may be processed or stored outside your province or outside Canada and may be accessible under the laws of those places. The operator must review Formspree's current terms, privacy practices, security settings, and retention controls before launch.
Possible sharing
The operator may need to share limited information with service providers and, only when appropriate for an introduction, with a prospective participant. The final policy must accurately describe what requester information is shared, when consent is obtained, and whether requesters can approve the information before an introduction.
Personal information should not be sold. Mobile opt-in information and SMS consent should not be shared for another party's independent marketing.
Amazon.ca wishlists
If you provide a wishlist link, the information visible through that list is controlled by your Amazon settings and Amazon's own practices. Review the list while signed out before sharing it. Lifeline US is not responsible for Amazon's privacy, account, checkout, or delivery systems.
SMS and email choices
SMS consent is optional and separate from permission to respond by email. If you opt in, your number may be used for conversational and request-related messages. You may revoke SMS consent by replying STOP or by emailing info@lifelineus.org. Any promotional or commercial electronic messages must use the consent and unsubscribe practices required by applicable law.
Retention and security
The operator must set and publish a retention schedule, restrict access, protect accounts with appropriate security controls, and delete information when it is no longer reasonably needed, subject to legal or safety requirements. No website or transmission method can guarantee absolute security.
Your choices
To ask about, correct, withdraw consent for, or request deletion of information you submitted, email info@lifelineus.org. The final notice must identify the accountable person and explain any rights and complaint process required by applicable law.
Updates
The published notice should identify the operator and effective date and should be updated whenever the form, matching process, messaging program, service providers, retention practices, or legal obligations change.
